V\u0161echny tyto l\u00e1tky pot\u0159ebujeme k norm\u00e1ln\u00edmu fungov\u00e1n\u00ed, ale samoz\u0159ejm\u011b ve vyv\u00e1\u017een\u00e9m mno\u017estv\u00ed. Ka\u017ed\u00fd z n\u00e1s m\u00e1 tak\u00e9 jin\u00e9 pot\u0159eby a podm\u00ednky. Na to, co a jak j\u00edme, maj\u00ed vliv faktory jako chu\u0165, t\u011blesn\u00e1 hmotnost, pohlav\u00ed, chu\u0165 k j\u00eddlu, pohybov\u00e9 n\u00e1vyky, vzd\u011bl\u00e1n\u00ed, rodinn\u00e9 vztahy, povol\u00e1n\u00ed, zdrav\u00ed, geny atd.. Obecn\u00e9 v\u00fd\u017eivov\u00e9 rady v \u010dasopisech nebo na internetu proto obvykle dlouhodob\u011b nefunguj\u00ed. V posledn\u00edch letech se v m\u00e9di\u00edch objevily takzvan\u00e9 FODMAP (nebo spr\u00e1vn\u011bji low-FODMAP) diety a FODMAP ve strav\u011b byly ozna\u010deny za mo\u017en\u00e9 vin\u00edky, pokud jde o zp\u016fsobov\u00e1n\u00ed probl\u00e9m\u016f s IBS (syndrom dr\u00e1\u017ediv\u00e9ho tra\u010dn\u00edku), jako je plynatost a pr\u016fjem. Co jsou to ale FODMAP? V tomto \u010dl\u00e1nku vysv\u011btl\u00edme, co jsou to FODMAP a co je d\u016fle\u017eit\u00e9 v\u011bd\u011bt, pokud jde o takzvan\u00e9 FODMAP diety (diety s n\u00edzk\u00fdm obsahem FODMAP).<\/p>\n<h2>IBS a dietn\u00ed poradenstv\u00ed<\/h2>\n<p>Probl\u00e9my s IBS se ji\u017e dlouho v\u00edce \u010di m\u00e9n\u011b \u00fasp\u011b\u0161n\u011b l\u00e9\u010d\u00ed pomoc\u00ed r\u016fzn\u00fdch dietn\u00edch doporu\u010den\u00ed. Je b\u011b\u017en\u00e9, \u017ee pacienti, kter\u00fdm l\u00e9ka\u0159 diagnostikoval IBS, dostanou tak\u00e9 doporu\u010den\u00ed nebo doporu\u010den\u00ed nav\u0161t\u00edvit dietologa, aby jim prov\u011b\u0159il stravu. Vych\u00e1zej\u00edce ze stravovac\u00edch n\u00e1vyk\u016f ka\u017ed\u00e9ho \u010dlov\u011bka, m\u016f\u017ee dietolog navrhnout ur\u010dit\u00e9 \u00fapravy stravy, o kter\u00fdch se p\u0159edpokl\u00e1d\u00e1, \u017ee p\u016fsob\u00ed na sn\u00ed\u017een\u00ed pot\u00ed\u017e\u00ed zp\u016fsoben\u00fdch nap\u0159\u00edklad plynatost\u00ed, z\u00e1cpou, pr\u016fjmem atd. Obvykl\u00e1 doporu\u010den\u00ed se t\u00fdkaj\u00ed p\u0159\u00edjmu vl\u00e1kniny nebo vynech\u00e1n\u00ed \u010di omezen\u00ed ur\u010dit\u00fdch potravin \u010di slo\u017eek potravin. V posledn\u00ed dob\u011b jsou FODMAP ozna\u010dov\u00e1ny za mo\u017en\u00e9 vin\u00edky a \u010dasto jsou prezentov\u00e1ny jako &#8222;rychl\u00e9 \u0159e\u0161en\u00ed&#8220; probl\u00e9m\u016f s IBS. Existuj\u00ed dokonce i ti, kte\u0159\u00ed prod\u00e1vaj\u00ed FODMAP potraviny a FODMAP rady nebo FODMAP diety r\u016fzn\u00e9ho druhu. Funguje v\u0161ak dieta FODMAP? Dr\u017een\u00ed diety FODMAP nevyl\u00e9\u010d\u00ed IBS, ale m\u016f\u017ee pomoci t\u011bm, kte\u0159\u00ed maj\u00ed v sou\u010dasn\u00e9 dob\u011b ve strav\u011b vysok\u00fd p\u0159\u00edjem FODMAP.<\/p>\n<h2>Sacharidy: d\u016fle\u017eit\u00e1 sou\u010d\u00e1st na\u0161\u00ed stravy<\/h2>\n<p>Sacharidy ve strav\u011b mohou m\u00edt r\u016fznou podobu. Nejjednodu\u0161\u0161\u00ed sacharidy se naz\u00fdvaj\u00ed jednoduch\u00e9 cukry neboli monosacharidy a pat\u0159\u00ed mezi n\u011b nap\u0159\u00edklad gluk\u00f3za a frukt\u00f3za. Dva monosacharidy mohou dohromady tvo\u0159it tzv. disacharidy, mezi kter\u00e9 pat\u0159\u00ed b\u011b\u017en\u00fd stoln\u00ed cukr (sachar\u00f3za) a lakt\u00f3za. V\u00edce ne\u017e dva monosacharidy mohou tak\u00e9 tvo\u0159it dlouh\u00e9 \u0159et\u011bzce slo\u017eit\u011bj\u0161\u00edch cukr\u016f, kter\u00e9 mohou b\u00fdt tak\u00e9 rozv\u011btven\u00e9. Ty se naz\u00fdvaj\u00ed polysacharidy (nap\u0159\u00edklad \u0161krob a celul\u00f3za). Mono- a disacharidy maj\u00ed sladkou chu\u0165 a t\u011blo je snadno vst\u0159eb\u00e1v\u00e1 jako energii (rychl\u00e9 sacharidy), zat\u00edmco polysacharidy sladkou chu\u0165 nemaj\u00ed a vst\u0159eb\u00e1vaj\u00ed se pomaleji nebo se nevst\u0159eb\u00e1vaj\u00ed v\u016fbec, ale proch\u00e1zej\u00ed celou cestu tr\u00e1vic\u00edm traktem (nap\u0159. tzv. vl\u00e1knina).<\/p>\n<p>Jednoduch\u00e9 i slo\u017een\u00e9 cukry jsou pro n\u00e1s velmi d\u016fle\u017eit\u00e9. Dod\u00e1vaj\u00ed n\u00e1m energii a dob\u0159e funguj\u00edc\u00ed tr\u00e1vic\u00ed trakt. Jde v\u0161ak o rovnov\u00e1hu a probl\u00e9my n\u00e1m m\u016f\u017ee zp\u016fsobit jak p\u0159\u00edli\u0161 mnoho, tak p\u0159\u00edli\u0161 m\u00e1lo sacharid\u016f. Na\u0161e modern\u00ed strava je v\u0161ak obvykle bohat\u00e1 na sacharidy a probl\u00e9my s jejich nadm\u011brnou konzumac\u00ed jsou nej\u010dast\u011bj\u0161\u00ed, nap\u0159\u00edklad v d\u016fsledku obezity.<\/p>\n<h2>Co je FODMAP?<\/h2>\n<p>FODMAP (Fermentable Oligo-saccharides, Di-saccharides, Mono-saccharides And Polyols) jsou sacharidy s kr\u00e1tk\u00fdmi cukern\u00fdmi \u0159et\u011bzci, kter\u00e9 se v na\u0161ich st\u0159evech \u0161patn\u011b vst\u0159eb\u00e1vaj\u00ed. Ty naopak mohou zv\u00fd\u0161it vylu\u010dov\u00e1n\u00ed tekutin do st\u0159eva a bakterie (kter\u00e9 lze samoz\u0159ejm\u011b koupit jako probiotika) mohou FODMAP vyu\u017e\u00edt jako zdroj energie prost\u0159ednictv\u00edm fermentace (kvasn\u00e9ho procesu). B\u011bhem kva\u0161en\u00ed vznikaj\u00ed kr\u00e1tk\u00e9 mastn\u00e9 kyseliny a plyny. \u00da\u010dinky FODMAP jsou p\u0159irozen\u00e9, vyskytuj\u00ed se u v\u0161ech lid\u00ed a pravd\u011bpodobn\u011b maj\u00ed svou funkci samy o sob\u011b, i kdy\u017e pacienti s IBS mohou m\u00edt probl\u00e9my mimo jin\u00e9 se zv\u00fd\u0161enou tvorbou plyn\u016f, a to i v p\u0159\u00edpad\u011b, \u017ee je strava nadm\u011brn\u011b bohat\u00e1 na FODMAP.<\/p>\n<p>P\u0159esto\u017ee si t\u00e9m\u011b\u0159 ka\u017ed\u00e1 \u0161v\u00e9dsk\u00e1 rodina v p\u00e1tek ve\u010der sedne a d\u00e1 si tacos a hranolky, na\u0161e stravovac\u00ed n\u00e1vyky se li\u0161\u00ed, a to nejen mezi r\u016fzn\u00fdmi kulturami a zem\u011bmi. V Austr\u00e1lii se obecn\u011b uk\u00e1zalo, \u017ee p\u0159\u00edjem FODMAP je vysok\u00fd, a tak\u00e9 se poda\u0159ilo prok\u00e1zat, \u017ee lid\u00e9, kte\u0159\u00ed sn\u00ed\u017eili p\u0159\u00edjem FODMAP, m\u011bli men\u0161\u00ed probl\u00e9my s IBS.<\/p>\n<h2>R\u016fzn\u00e9 FODMAP<\/h2>\n<p>FODMAP se skl\u00e1daj\u00ed z n\u011bkolika r\u016fzn\u00fdch skupin fermentovateln\u00fdch sacharid\u016f. My lid\u00e9 je nedok\u00e1\u017eeme rozlo\u017eit, proto\u017ee n\u00e1m k tomu chyb\u00ed pot\u0159ebn\u00e9 enzymy. Z\u016fst\u00e1vaj\u00ed tedy ve st\u0159evech a jejich nadm\u011brn\u00e9 mno\u017estv\u00ed m\u016f\u017ee zp\u016fsobit pot\u00ed\u017ee. Disacharidy, jako je lakt\u00f3za, mohou b\u00fdt FODMAP, ale pouze v p\u0159\u00edpad\u011b, \u017ee chyb\u00ed dostate\u010dn\u00e9 mno\u017estv\u00ed enzymu lakt\u00e1zy v pom\u011bru k po\u017eit\u00e9mu mno\u017estv\u00ed. Mno\u017estv\u00ed lakt\u00e1zy, a t\u00edm i na\u0161e tolerance lakt\u00f3zy, se li\u0161\u00ed. Monosacharidy mohou b\u00fdt tak\u00e9 FODMAP, pokud nemohou b\u00fdt v dostate\u010dn\u00e9 m\u00ed\u0159e vst\u0159eb\u00e1ny. Nap\u0159\u00edklad frukt\u00f3za se m\u016f\u017ee vst\u0159eb\u00e1vat ve st\u0159ev\u011b, ale jej\u00ed transport je z\u00e1visl\u00fd na p\u0159\u00edtomnosti gluk\u00f3zy. Proto je nutn\u00e9 zv\u00e1\u017eit p\u0159\u00edjem frukt\u00f3zy s jin\u00fdmi potravinami s gluk\u00f3zou, aby se sn\u00ed\u017eilo mno\u017estv\u00ed frukt\u00f3zy, kter\u00e9 z\u016fst\u00e1v\u00e1 ve st\u0159ev\u011b. Vzhledem k tomu, \u017ee dochucov\u00e1n\u00ed a zpracov\u00e1n\u00ed potravin se li\u0161\u00ed, m\u016f\u017ee se obsah FODMAP v t\u011bchto potravin\u00e1ch ve \u0160v\u00e9dsku li\u0161it.<\/p>\n<p>Oligosacharidy (FOS a GOS) se nach\u00e1zej\u00ed v \u017eit\u011b a \u017eitn\u00fdch v\u00fdrobc\u00edch (nap\u0159. \u017eitn\u00fd chl\u00e9b), p\u0161enici a p\u0161eni\u010dn\u00fdch v\u00fdrobc\u00edch (nap\u0159. p\u0161eni\u010dn\u00fd chl\u00e9b, t\u011bstoviny, kuskus, p\u0161eni\u010dn\u00e9 otruby), broskv\u00edch, persimonech, melounech, arty\u010doc\u00edch a r\u016fzn\u00fdch lu\u0161t\u011bnin\u00e1ch (fazole a \u010do\u010dka), cibuli, \u010desneku.<\/p>\n<p>Disacharid lakt\u00f3za se nach\u00e1z\u00ed v ml\u00e9ce a jogurtov\u00fdch v\u00fdrobc\u00edch.<\/p>\n<p>Monosacharid frukt\u00f3za se v nadbytku oproti mno\u017estv\u00ed gluk\u00f3zy nach\u00e1z\u00ed v t\u0159e\u0161n\u00edch, mangu, konzervovan\u00e9m ovoci, melounu, ovocn\u00fdch \u0161\u0165\u00e1v\u00e1ch a su\u0161en\u00e9m ovoci, ch\u0159estu, arty\u010dok\u00e1ch, cukrov\u00e9m hr\u00e1\u0161ku, medu a kuku\u0159i\u010dn\u00e9m sirupu.<\/p>\n<p>Polyoly, jako je sorbitol, se nach\u00e1zej\u00ed v jablk\u00e1ch, hru\u0161k\u00e1ch (jable\u010dn\u00e1\/hru\u0161kov\u00fd d\u017eus), meru\u0148k\u00e1ch, hru\u0161k\u00e1ch, nektarink\u00e1ch a manitol v kv\u011bt\u00e1ku, houb\u00e1ch a melounu. Pokud v\u00edte, \u017ee m\u00e1te vysok\u00fd p\u0159\u00edjem v\u00fd\u0161e uveden\u00fdch potravin, jako je cibule, \u010desnek, jable\u010dn\u00fd d\u017eus, meloun, konzervovan\u00e9 ovoce, med atd., m\u016f\u017eete se je pokusit trochu omezit. Nemus\u00edte chodit k drah\u00e9mu soukrom\u00e9mu poradci a dietolog v m\u00edstn\u00edm zdravotn\u00edm st\u0159edisku v\u00e1m jako pacientovi s IBS s takovou dietou jist\u011b pom\u016f\u017ee, pokud to budete cht\u00edt zkusit.<\/p>\n<p>Nespol\u00e9hejte v\u0161ak na &#8222;rychlou n\u00e1pravu&#8220; a nezapom\u00ednejte, \u017ee c\u00edlem diety FODMAP nen\u00ed vylou\u010dit FODMAP nav\u017edy, ale sn\u00ed\u017eit jejich nadm\u011brn\u00fd p\u0159\u00edjem. Vylou\u010den\u00e9 FODMAP jsou pak znovu zavedeny v tolerovan\u00e9 m\u00ed\u0159e. Je to \u010d\u00e1ste\u010dn\u011b proto, \u017ee nechcete dlouhodob\u011b ovliv\u0148ovat variabilitu stravy nebo obsah \u017eivin, krom\u011b toho byly po dlouhodob\u00e9 diet\u011b FODMAP pozorov\u00e1ny n\u011bkter\u00e9 znepokojiv\u00e9 negativn\u00ed \u00fa\u010dinky na slo\u017een\u00ed st\u0159evn\u00ed fl\u00f3ry.
